We are upgrading our Patient Portal!

FCS is converting our Electronic Health Record from Paragon to Cerner on August 20, 2018. The Patient Portal will also be transitioned to Cerner at this time. During the transition, April 1 – August 20, new data will not be available in the Relay Patient Portal. Any results or information completed prior to April 1 can be accessed using your Relay Health ID and password. After August 20, 2018, new data will be available in the new, easy to use Cerner Portal!

How will I receive my results?

You will be notified by phone or mail. If you have not been notified within 10 days, please call 402.372.2477. If you wish to have a copy of the results, please contact the HIM Department at 402.372.6724.

Q: What will happen to the patient records after March 31, 2018?

A: The patient will continue to have access to historical date until September 28, 2018. During this time, it is recommended that patients download any health information they would like to keep. Patients can save data to their computer, print it out or email it.

A: Patients will continue to access the Portal via the RelayHealth link https://app.relayhealth.com
